Not holding train for crew rest (30 Minutes)
According to the Denver-Sterling agreement, if a train is not held for 30 minutes for crew rest and there is no delay to the train, it is a claim. Below is an example of the claim to be submitted.
ALLOW 130.O MILES ACCOUNT BOARD RUNAROUND. ON MM/DD/YYYY I WAS FIRST OUT ON DENVCO BOARD 410 AND SHOULD HAVE BEEN CALLED FOR *train* ON DUTY AT *time*. I WAS RESTED AT *time*. THE *train* DEPARTED AT *time*. PER THE DENVER TO STERLING ID AGREEMENT, SEC 2A, "TRAINS WILL BE HELD FOR 30 MINUTES FOR CREW REST IF THERE WILL BE NO DELAY ON THE TRAIN". TRAIN *train* DEPARTED AT *time*. PLEASE ALLOW FOR 130.O MILES FOR THE MISHANDLING.

On the remarks section, put description of problem. Items like stinky toilet, cab seal, broken cooler will get declined. Be more descriptive with so that a mechanical facility can go directly to issue and repair.
toilet: is full/overflowing, feces outside of seat, will not flush, etc
Cooler: Cooler is not keeping water cool
Cab Seal: annotate temperature outside (below 50 degrees) and where seal is broken/missing
